Judges Disclosure and Agreement Forms - Deadline

WPCS PANEL JUDGES - Have you returned your Judges forms?

Early August two important forms were sent to you

- Self-disclosure form

- Judge's agreement form.

Both documents must be completed, signed by yourself, dated and returned to the WPCS Office, either via email (enquiries@wpcs.uk.com) or post by no later than 30th SEPTEMBER 2025. If you are attending this weekends judge's conference you can also hand them in here.

If you choose to post the forms, we recommend returning these via recorded delivery if sending in the post so that you have proof of submission, please retain the tracking number for your records as this may be required if your form is not received by the office.

If you have not received your forms or have misplaced them please contact the office for duplicate forms.

Unfortunately, if we do not receive both signed forms by the deadline then your name will not be listed in the 2026 handbook and therefore you will not be listed as a Panel judge for 2026.
