Judges Disclosure and Agreement Forms 2027
Current WPCS Judges can now complete their annual Judges disclosure and agreement forms online. Simply complete both of the below forms and submit them once fully completed. These will then be sent electronically direct to the WPCS office, with no need for postage and no risk of things going missing in the post.
Please make sure both forms are completed or we will be unable to include you on the judges list for 2027.

All forms must be fully completed and submitted by no later than midnight on
Wednesday 30th September 2026.

If you would prefer to complete a paper form these can be downloaded and printed below. If returning these via post please make sure you send them tracked delivery and retain the proof of postage. As should they go missing you will require this to remain on the WPCS panel of judges.
